Birthparent Printable Form

your donations to the Kentucky Adoption Reunion Registry are greatly appreciated

If you had a child born before 1970 then you can look in the Ky Birth Books for the certificate number  - which is in Alpha order by the last name.  If you have a certificate number then that number will match with
the current birth index and give you the adopted name.

Make sure you have placed a letter in the file with the agency as well as the state that gives your permission for contact

Write to the hospital where the delivery occurred to get a copy of your medical records (birth mothers are entitled to their medical records)

If you have been contacted by the state  that the adoptee has filed the petition and wants contact - you must send a letter back to the state that gives permission.

If the birth family has a heredity genetic disease with medical proof/statements from physicians - this information with the approval of the state can be passed on to the adoptee. (Medical - not personal info of the birth family, such as name, address)

Adoption Term - Lingo
