Can't get to the library to look up your birth name?  Check here for order form

There are black birth books in the libraries that list the birth name for births from 1912 thru 1969 (usually 2 volumes for each year - A-K and L-Z).

Above is a sample page out of the birth index book.
In the far left hand side is a column marked Certificate
number.  This number does not change.  So by taking the certificate number from your adopted birth certificate and going thru the book for the year of your birth (could vary in some years) you can match up the number and it will have your birth mother's maiden name and the name you were given at birth.   If in the book under child name it shows a number 1 or 2 as a middle initial - 1 is for a male child and 2 is for female child.

The certificate number is up in the upper right hand corner of the certified certificate from the state - and usually starts with 116-XX_XXXXX  (the last five numbers is the certificate number that you will match up.   If you can not find your certificate number, a lot of times the library has microfiche cards that the adopted name is on and the certificate number is listed there.     Can't find your number?  - email us.
